The MolinaCares Accord, in collaboration with Senior Whole Health by Molina Healthcare, is proud to present a virtual panel examining how the Place-Based Supports (PBS) approach improves health outcomes for seniors.
Nearly 50% of older adults living alone lack the financial resources required to pay for basic needs including food, medicine, clothing, and hygiene products. To meet the unique needs of seniors, MassHealth has introduced the PBS Initiative – an approach to care integrating community health and well-being strategies into Senior Affordable Housing Sites.
We convened a panel of experts to address the importance of food security, crisis intervention, behavioral health, the integration of resident and service providers into specialty housing, and more through the lens of the PBS model.
