Molina Healthcare of Ohio Donates $10,000 to Support Population Health Management Strategy at New Senior Living Housing Facility

COLUMBUS, Ohio, Feb. 13, 2026 – Molina Healthcare of Ohio (“Molina”) presented a $10,000 grant to the National Church Residences Foundation to help fund a network of onsite health and wellness supports in the new Berwyn East Place, a senior housing community located on East Livingston Avenue.
This population health management model is unique in the affordable housing industry and is aimed at reducing hospital visits and costly medical interventions through early identification and treatment of conditions, preventative care, social interaction, and other proven wellness strategies. The goal is to keep seniors living independently in the community for as long and safely possible.
The grant agreement fosters collaboration among Molina, National Church Residences and community partners to increase access to clinical care, community resources, and essentials, all of which can positively impact health outcomes. Berwyn East Place will make available to residents an on-site nurse practitioner, behavioral health worker, community health worker, and personal care aid as well as move-in kits and computer access and training.

Berwyn East Place is a new affordable senior housing facility owned by National Church Residences. It has 88, 1-bedroom apartments and will open later this month. The new community will be available to individuals 55+ who meet income limits. Its focus is the coordination of community resources to address the many health challenges of its residents.
